Each team participating in the 2019 YMCA National Diving Championship must have a certificate of Insurance on file at the YMCA of the USA offices in Chicago, Illinois. The certificate must be valid from the first day of the Diving Championship through the last day of the Championship.

Email of a certificate may be sent to Sam Mars at sam.mars@ymca.net. Certificate of Insurance may be faxed to 312-977-1134, HOWEVER, THE ORIGINAL MUST STILL BE SENT to YMCA of the USA, Attn: Insurance Manager, 101 North Wacker Dr., Chicago IL 60606. ....More
